Consent To Tattoo a Minor
Tattooing for minors
(age 16 through 17 Years old)
Florida consent for tattooing a minor form must be filled out and notarized the day of the tattoo. A link to the form can be found HERE or printed from the Florida Health Department website.
Minor and parent/legal guardian must have government-issued photo ID and be present at the time service is provided.
Artists will not tattoo certain areas of the body on a minor.
Some artists WILL NOT tattoo minors. Please check with your artist.
